Wall Certificate and Letter of Certification

The Louisiana Department of Insurance no longer mails or otherwise produces Wall Certificates other than via this online form. However, the Department will still continue to accept written requests for paper Letters of Certification along with the $3.00 fee as we fully understand that some producers/adjusters may not have access to technology. We have sent out the announcement of the online letter of certification to all other states and have requested that they accept this PDF letter of certification.
 

  1. The Louisiana Department of Insurance cannot accept requests for Wall Certificates to be e-mailed or otherwise produced by this department.
     
  2. Please read the section below for a brief explanation on how to use this form.
     

Important! Please Read:

This Certificate Program now generates a Wall Certificate and a Letter of Certification in Adobe PDF Format. 

  1. Adobe Acrobat Reader is required to view your certificate. Many computers already have this installed, and you probably do not need to install anything. Please attempt to create a certificate before attempting to download any programs on your computer.  If necessary, you may download this from Adobe with the preceeding link.
     
  2. If you do not have Adobe Acrobat Reader installed, you may still download your certificate for later viewing. You will be prompted to download and save a file. Please place this file in a location on your computer where you will be able to find it. Once you have Adobe Acrobat Reader installed, you may open the file.
     
  3. The certificate image is approximately 450Kb in size. It may take anywhere from 1 to 10 minutes to download the image, depending on your internet connection speed. Even if your browser says 'done', it is still downloading.
